HouseCalls supports members and health plans

Undiagnosed health conditions may lead to poor outcomes for members and costly outcomes for health plans. HouseCalls can aid in:

  • Gaining a broader understanding of a member’s care needs
  • Decreasing the risk of serious health events
  • Preventing unnecessary hospital admissions

During the visit members benefit from:

  • 45 to 60 minutes of one-on-one time with an Advanced Practice Clinician in the convenience of their own home
  • A head-to-toe exam, medication reconciliation and appropriate health screenings
  • Educational materials and resources tailored to their healthcare needs
